Winds of Agape Licensed Home Care Agency strives to provide excellent, compassionate care given by our professionally and specially trained staff of Certified Home Health Aides (CHHAs) and Personal Care Aides (PCAs).
Our Licensed Home Care Agency serves the following six counties of Central New York: Cayuga County, Cortland County, Madison County, Oneida County, Onondaga County and Oswego County.

NHTD Waiver
The Nursing Home Transition and Diversion (NHTD) Medicaid Waiver is a Home and Community Based Services (HCBS) program, administered by the New York State Department of Health (DOH) through contractual agreements.
The NHTD waiver uses Medicaid funding to provide supports and services to assist individuals with disabilities and seniors toward successful inclusion in the community. Waiver participants may come from a nursing facility or other institution (transition), or choose to participate in the waiver to prevent institutionalization (diversion).

Private Duty Care
Patient receives nursing services at home from Registered Nurses (RNs) or Licensed Practical Nurses (LPNs). Patient may receive continuous nursing services beyond the scope of care available from certified home health care agencies (CHHAs).
For Medicaid-eligible recipients, after a physician provides written orders or a letter of medical justification, a certified home health agency or a local social services office provides a written assessment of the need for private duty nursing and authorizes a specific number of hours of care. This program is available through Medicaid, private payment, and some health insurers.

TBI Waiver
The Traumatic Brain Injury (TBI) Waiver is administered centrally by the New York State Department of Health (DOH) and implemented through Regional Resource Development Centers (RRDC).
The waiver uses Medicaid funding to provide supports and services to assist individuals with a traumatic brain injury (TBI) toward successful inclusion in the community. Waiver participants may choose to move into the community from a nursing facility. Others may choose to participate in the waiver to prevent unnecessary institutionalization.

Veteran's Association
The program supports Veterans living at home instead of being placed into a community living center or nursing home. The types of services provided include assistance with bathing, dressing, eating, help in using the toilet and getting in or out of bed or a wheelchair.
Other services can include help with walking, exercises, using medical equipment, and health monitoring. In some cases, specific household tasks such as light housekeeping may be provided following personal care.
